Cleveland, Ohio clinic performs US’s first face transplant

A team of eight transplant surgeons in Cleveland Clinic in Ohio, USA, led by reconstructive surgeon Dr. Maria Siemionow, age 58, have successfully performed the first almost total face transplant in the US, and the fourth globally, on a woman so horribly disfigured due to trauma, that cost her an eye. Two weeks ago Dr. Siemionow, in a 23-hour marathon surgery, replaced 80 percent of her face, by transplanting or grafting bone, nerve, blood vessels, muscles and skin harvested from a female donor’s cadaver.

The Clinic surgeons, in Wednesday’s news conference, described the details of the transplant but upon request, the team did not publish her name, age and cause of injury nor the donor’s identity. The patient’s family desired the reason for her transplant to remain confidential. The Los Angeles Times reported that the patient “had no upper jaw, nose, cheeks or lower eyelids and was unable to eat, talk, smile, smell or breathe on her own.” The clinic’s dermatology and plastic surgery chair, Francis Papay, described the nine hours phase of the procedure: “We transferred the skin, all the facial muscles in the upper face and mid-face, the upper lip, all of the nose, most of the sinuses around the nose, the upper jaw including the teeth, the facial nerve.” Thereafter, another team spent three hours sewing the woman’s blood vessels to that of the donor’s face to restore blood circulation, making the graft a success.

The New York Times reported that “three partial face transplants have been performed since 2005, two in France and one in China, all using facial tissue from a dead donor with permission from their families.” “Only the forehead, upper eyelids, lower lip, lower teeth and jaw are hers, the rest of her face comes from a cadaver; she could not eat on her own or breathe without a hole in her windpipe. About 77 square inches of tissue were transplanted from the donor,” it further described the details of the medical marvel. The patient, however, must take lifetime immunosuppressive drugs, also called antirejection drugs, which do not guarantee success. The transplant team said that in case of failure, it would replace the part with a skin graft taken from her own body.

Dr. Bohdan Pomahac, a Brigham and Women’s Hospital surgeon praised the recent medical development. “There are patients who can benefit tremendously from this. It’s great that it happened,” he said.

Leading bioethicist Arthur Caplan of the University of Pennsylvania withheld judgment on the Cleveland transplant amid grave concerns on the post-operation results. “The biggest ethical problem is dealing with failure — if your face rejects. It would be a living hell. If your face is falling off and you can’t eat and you can’t breathe and you’re suffering in a terrible manner that can’t be reversed, you need to put on the table assistance in dying. There are patients who can benefit tremendously from this. It’s great that it happened,” he said.

Dr Alex Clarke, of the Royal Free Hospital had praised the Clinic for its contribution to medicine. “It is a real step forward for people who have severe disfigurement and this operation has been done by a team who have really prepared and worked towards this for a number of years. These transplants have proven that the technical difficulties can be overcome and psychologically the patients are doing well. They have all have reacted positively and have begun to do things they were not able to before. All the things people thought were barriers to this kind of operations have been overcome,” she said.

The first partial face transplant surgery on a living human was performed on Isabelle Dinoire on November 27 2005, when she was 38, by Professor Bernard Devauchelle, assisted by Professor Jean-Michel Dubernard in Amiens, France. Her Labrador dog mauled her in May 2005. A triangle of face tissue including the nose and mouth was taken from a brain-dead female donor and grafted onto the patient. Scientists elsewhere have performed scalp and ear transplants. However, the claim is the first for a mouth and nose transplant. Experts say the mouth and nose are the most difficult parts of the face to transplant.

In 2004, the same Cleveland Clinic, became the first institution to approve this surgery and test it on cadavers. In October 2006, surgeon Peter Butler at London‘s Royal Free Hospital in the UK was given permission by the NHS ethics board to carry out a full face transplant. His team will select four adult patients (children cannot be selected due to concerns over consent), with operations being carried out at six month intervals. In March 2008, the treatment of 30-year-old neurofibromatosis victim Pascal Coler of France ended after having received what his doctors call the worlds first successful full face transplant.

Ethical concerns, psychological impact, problems relating to immunosuppression and consequences of technical failure have prevented teams from performing face transplant operations in the past, even though it has been technically possible to carry out such procedures for years.

Mr Iain Hutchison, of Barts and the London Hospital, warned of several problems with face transplants, such as blood vessels in the donated tissue clotting and immunosuppressants failing or increasing the patient’s risk of cancer. He also pointed out ethical issues with the fact that the procedure requires a “beating heart donor”. The transplant is carried out while the donor is brain dead, but still alive by use of a ventilator.

According to Stephen Wigmore, chair of British Transplantation Society’s ethics committee, it is unknown to what extent facial expressions will function in the long term. He said that it is not certain whether a patient could be left worse off in the case of a face transplant failing.

Mr Michael Earley, a member of the Royal College of Surgeon‘s facial transplantation working party, commented that if successful, the transplant would be “a major breakthrough in facial reconstruction” and “a major step forward for the facially disfigured.”

In Wednesday’s conference, Siemionow said “we know that there are so many patients there in their homes where they are hiding from society because they are afraid to walk to the grocery stores, they are afraid to go the the street.” “Our patient was called names and was humiliated. We very much hope that for this very special group of patients there is a hope that someday they will be able to go comfortably from their houses and enjoy the things we take for granted,” she added.

In response to the medical breakthrough, a British medical group led by Royal Free Hospital’s lead surgeon Dr Peter Butler, said they will finish the world’s first full face transplant within a year. “We hope to make an announcement about a full-face operation in the next 12 months. This latest operation shows how facial transplantation can help a particular group of the most severely facially injured people. These are people who would otherwise live a terrible twilight life, shut away from public gaze,” he said.

Black Lives Matter protest causes delays at London City Airport

Flights were disrupted at London’s City Airport today by activists from the UK branch of the Black Lives Matter movement who occupied the runway at the airport. Inbound flights were diverted to either Gatwick or Southend while outbound flights from City experienced considerable delays.

The Metropolitan Police, who arrived at the airport at 05:40 local time (0440 UTC), gave a further description: “There are currently nine protesters on the runway at the airport. They have erected a tripod and have locked themselves together.”

Black Lives Matter activists claimed responsibility via their Twitter account, saying they intended to shut the airport down “to protest the UK’s environmental impact on black people”, claiming the “climate crisis is a racist crisis” and contrasting the “small elite” who fly from City Airport to the “migrants have died or gone missing in the Mediterranean”.

In a further statement from the activists, they contrasted the expansion plans at City Airport with the lives of poor people in the surrounding borough, Newham: “Recently London City Airport was given approval to expand its capacity, a move that consigns the local community in Newham to further deterioration of their environment. The average salary of a London City Airport user is €136,000 euros and 63% of them work in business, finance or other business services. It is an airport designed for the wealthy. At the same time 40% of Newham’s population struggle to survive on £20,000 or less. When black people in Britain are 28% more likely to be exposed to air pollution than their white counterparts, we know that environmental inequality is a racist crisis.”

US Nationwide Pollution Permit Restrictions Upheld

The US Army Corps of Engineers decision to place restrictions on issuance of nationwide pollution permits has been upheld by a federal court. In National Association of Home Builders v. Army Corps of Engineers, the District Court for the District of Columbia found that the Corps of Engineers had not acted in an “arbitrary” or “capricious” manner in changing the terms and conditions for issuance of a national pollution permit, including reducing the size of area into which pollutants may be discharged from 10 acres to 1 acre, raising the threshold for requiring additional permits from 1 acre to 1/10 acre,

A nationwide permit allows an organization to engage in certain industrial activities on a national basis (such as mining and construction), reducing the amount of paperwork and filings needed for otherwise minor environmental impacts, as opposed to an ordinary permit for a specific location which will engage in activities which generate water pollution.

Due to concerns over the amount of discharge taking place in waterways, the Corps of Engineers began in the 1980s to reduce the authority granted by nationwide permits and to bar use of the permits in certain ecologically sensitive areas.

Some industry groups, including the plaintiff in the above case, The National Association of Home Builders, sued the Corps of Engineers in 2000 over the change in an attempt to block its implementation. Environmental groups, including the Sierra Club, were given permission to intervene in the case in support of the actions of the Corps.

Environmental groups were pleased with the decision, but are concerned over other actions of the Bush Administration, such as the attempts to weaken provisions of the 2002 Clean Water Act to allow additional dumping of construction and mining waste into waterways as fill material.

Pre Construction Planning For A Renovation

When renovating a property, it is imperative that all necessary steps are taken to ensure the construction is done within the boundaries of the law, and in a way that will increase, not decrease, the equity of that home or building.

First, consider the value of the property you are renovating. With more stringent rules from banks for refinancing, it is important to have the property professionally appraised to find the amount of equity you have in the property. If you are not planning to refinance, you will still want to speak with an appraiser to be sure your plans will increase the value of the property, and not decrease your equity. A renovation should always increase the value of that building, so that if you one day decide to sell the home or building, you make back the money that was invested into the renovation.

Once you have established the types of renovations you would like to make, it is necessary to take a look at the property boundary lines for your particular city. If you are adding an addition, you’ll want to ask your local zoning board what the laws and the restrictions are for your area. They will need your exact address to help determine the zone you are in. Many towns have multiple zoning laws depending on what the surrounding area and landscape is which may restrict or forbid the type of renovation you are hoping for.

The style of the original building you are renovating also factors into the renovation planning. If you have a Victorian building, you will want to use extreme caution in choosing modern upgrades, as many of them will actually take away from the character of the house and decrease its value. Although some features such as plumbing, electrical, and heating/air conditioning will help to increase your property value, others can decrease your value. This is why speaking with your renovation expert will help you determine which features help or hinder the value of the building, and they can also advise you as to the proper materials to use.

You will also need to carefully consider the main objective of this renovation. Are you upgrading an old house to make it more comfortable and have more aesthetic value, or is the house in need of structural work? If it s possible the buildings safety has been compromised, you may need to hire an inspector to first evaluate the renovation project. There would be no benefit in starting cosmetic work if the structure is in need of repair.

As you walk through the pre-construction planning phase with your contractor, mention any concerns you have before the work starts, as many of the issues can be repaired at the beginning of the renovation which will save you time and money in the long run. At this point, you should also begin adding the costs of the materials needed with the rate of labor, or discussing the option of your contractor supplying all the materials.

If you decide to start your renovation project, you will need to apply for a building permit, or renovation permit; these are usually found at your town or city hall. When applying for any permits, speak to your contractor and state or local code enforcement to find out exactly what steps will need to be taken for the renovations. Some states will require a separate permit for water, electricity etc. So discussing the details with your contractor will allow you to have all permits applied for and received before the work begins. If you are building near wetlands or property lines you may also need a variance from your town or state and you will need these before the work is able to begin.

Early returns in Iraqi elections

The Independent Electoral Commission of Iraq (IECI) reported uncertified early returns in the Iraqi legislative election. The preliminary numbers indicate that religious parties have done well, winning up 80 percent of the vote in some provinces. In Baghdad province, the Shia United Iraqi Alliance leads with 58% of the vote. The United Alliance had received 1,398,778 votes, followed by the Sunni Arab Iraqi Accordance Front with 454,107 votes, and the Iraqi National List ticket of Iyad Allawi, the former prime minister, with 330,082 votes.

In the Kurdish dominated Northern provinces of w:Dohuk, w:Erbil, and w:Sulaymaniyah, the Kurdistan Coalition List had received between 87% and 95% of votes cast. In the southern province of Basrah the Shia United Iraqi Alliance held a commanding lead with 615,255 votes. Allawi’s group was in second place with 87,538 votes, while the Iraqi Accordance Front trailed with 37,098.

According to the IECI, these preliminary results are subject to the adjudication of complaints by the IECI Board of Commissioners. Following the adjudication of complaints, only results certified by the IECI Board of Commissioners will be considered as final. Election officials are investigating more than 1,000 complaints about irregularities, 20 of them considered serious. Final results will not be released until early January. Abdul Husain Hindawi, the head of the IECI dismissed calls for a re-vote by Adnan al-Dulaimi, one of the leaders of the National Concord Front coalition.

Ford offers US$78 million for Romanian auto plant

Ford Motor Company, the U.S. car maker, will reportedly pay €57 million (US$78 million) for a 72.4 percent stake in the Romanian assembly plant Automobile Craiova, a Romanian official said Friday.

“The offer of Ford Motor Company for a 72.4 percent stake is €4.1556 per share or €57 million overall,” said Sebastian Vladescu, head of the State Property Agency (AVAS), after opening Ford’s improved offer. Vladescu added that the contract may be signed on September 12, during the auto show in Frankfurt.

The Romanian government bought back the Craiova-based car maker from Daewoo Motors, in late 2006 for US$51 million. As the Korean company was bankrupt, the government had to pay another $10 million for debts stemming from past loans. Ford is the only bidder for the purchase of the factory.

According to Washington Post, many auto-part makers have set up in the new European Union member country, attracted by cheap labor, favourable tax rates and the rising output of Renault’s Dacia plant. The vice president of Dacia, Constantin Stroe, said that the price Ford offers is not important. “It’s important to have the factory working as soon as possible”, he added. “With this production facility, Romania will become an important auto production center in Europe”, concluded Stroe, cited by HotNews.
